SFC Program?

You’re eligible for our SFC program if you:

  • Care for someone that needs assistance in daily tasks, such as bathing, dressing, eating, and using the restroom that is unable to live alone or be left unattended, or who has a chronic illness or disability
  • Are a  caregiver to someone such as a parent, spouse, adult child, or sibling.
  • You live in the home with a family member.
  • You are willing to attend one training a month.
  • You don’t have employment outside of the home.
  • You’re able to complete weekly wellness notes.
  • Have a valid driver’s license and social security card.
